Saver Support

This section provides tools related to providing ongoing support of your participants in the program. To submit all completed forms:
  • Scan the forms
  • Rename the forms Participant LastName, Participant FirstName, Document Type, Date (example: Smith, John_LOA_1.31.14)
  • Emailing them to the documents link on the right ( using a secure network connection to protect sensitive information
The asset-specific Participant Development Plan must be completed by the participant and submitted to CASA preferably ten days prior to the first matched withdrawal. The Leave of Absence Request can be submitted as long as the participant has been enrolled in the program for at least six months. See the Member Handbook for important details. The Participant Transfer Form is used when a participant wants to transfer his or her IDA to a spouse or a dependent. This form must be submitted along with a new online application package and $25 application fee for the spouse or dependent. The Member Transfer Form is used to transfer a participant’s support to a different member organization than the enrolling member.
